I hiked up to Paleokastro from Mandraki town during our one week stay on Nisyros in August 2019. My first attempt was not successful, but the second time I followed the signs very carefully, they led through the narrow winding and steep streets of Mandraki village and finally to a dirt and stone path, which after maybe a mile reached the wide paved road very near to the castle ruins. Although the fort is barely more than a pile of rubble, the walls of a small section have been restored and it is worth a visit simply to contemplate the history. Plus, the views from there are extraordinary! There are several signs around detailing the restoration, with interesting before and after pictures.

The view of Kos from the top is incredible as well as the view of Mandraki. The hiking path is basically an uneven stone path lined with intermittent trees for shade. I suggest going in the early morning. There is a parking lot at the top if you'd like to drive. It looked like there was some sort of ticket booth, but it is either not operational any longer or it was only for the high season (I was there mid September). There are plaques telling the history. Most of the photos are of the view while standing on the wall of the fort.
